Do you know how to use the LCD guidelines to support terminality?

What kind of documentation is needed to assert a life expectancy of less than six months? Do you know the difference between terminal and chronic illnesses? This detailed webinar will teach you how to support terminality and hospice eligibility via thorough documentation and how to avoid related roadblocks.

AFTER THIS WEBINAR YOU’LL BE ABLE TO:
  • Identify documentation that supports physician CTI and life expectancy less than six months
  • Understand what measurable objectives support terminality and decline
  • Distinguish between chronic versus terminal illnesses
  • Explain the importance of congruent documentation in supporting terminality

WEBINAR DETAILS

Hospice documentation must be very descriptive to support the terminal diagnosis and terminality throughout the hospice course of care. This requires nurses and hospice teams to look at patient improvement and decline from visit to visit. Some items require frequent documentation such as mid-arm circumference (MAC)/weight, wound characteristics, cognition, ADLs, and self-care.

This webinar will address the hospice specifics and focus areas that should be addressed in documentation to support terminality and hospice eligibility. It will review chronic versus terminal status, standardized tools, congruent documentation, key focus areas (per the LCD guidelines), and hospice eligibility. In addition, it will break down the challenged documentation areas and how to avoid those challenges. This will be a very focused webinar, excellent for nursing staff, leadership, QA personnel, and any new clinicians.

THIS WEBINAR WILL BENEFIT THE FOLLOWING AGENCIES:

  • Hospice
  • Palliative Care

WHO SHOULD ATTEND?

This informative session is designed for all team members who document in the hospice clinical chart with a special focus on nursing (admission, case managers, LPN/LVN, staff RNs), medical directors, and the IDT. It is recommended that all agency leadership attend.
