The IDG and Plan of Care remains a key area of focus and challenge for agencies both with surveys and financial audits. Currently, many hospice agencies are under increased scrutiny and oversight with requests for additional documentation review that includes both IDG and POC documentation. This presentation will provide a step-by-step guide on how to conduct an effective IDG, including documentation to meet the Plan of Care requirement. We will review all aspects of incorporating assessments, patient visits (both routine and PRN), care coordination, and changes of condition into the Plan of Care. Updating and reviewing the POC per the regulations and what team members should be involved will also be covered. This is an excellent opportunity for nurses new to hospice, clinical supervision, and/or leadership to learn how to avoid the pitfalls of survey deficiencies and recoupment of Medicare dollars.
