CMS’s new HOPE instrument replaces the Hospice Item
Set (HIS). It is designed to help hospice agencies better understand patient
care needs through the dying process, ultimately contributing to the patient’s care
plan. The HOPE instrument will provide assessment-based quality data to
enhance the HQRP through standardized data collection and provide additional
clinical data that could inform future payment refinements. HOPE’s aim is to collect quality data throughout the
course of the patient’s stay by introducing additional timepoints called the
HOPE Update Visit (HUV). Hospice agencies will be required to submit up to two
HUVs, at specific times. Hospice workflow and the COPs will be considered with
the intent to improve quality efforts.
This webinar will introduce hospice agencies to the HOPE instrument.
It will cover the intent and purpose of the tool, along with anticipated implementation
timeframes. You’ll learn about the reliability and validity of the tool, public
reporting, and how to use it, including the required HOPE update visits and
additional timepoint assessments. This tool is intended to capture the needs of
the patient and family in real time, therefore impacting clinical
interventions. It is critical that agencies understand how to use this tool, including
when to assess (on admission) and what to assess at HUV timepoints.
