A series of three webinars!

One in four Medicare patients hospitalized for acute medical illness is discharged to a skilled-nursing facility or an assisted-living facility. Approximately 23% of these patients are readmitted to the hospital within 30 days. The care transition from hospital to skilled-nursing or assisted-living environments is often marked by disruptions in care and poor communication between hospital and post-acute providers. Hospice and home health providers can make a difference and position themselves as the provider of choice for all three parts of this healthcare continuum.

This series will provide a comprehensive overview of each environment, identify their needs, and address how home health and hospice can manage their challenges. If you are unsure how each type of organization is led and staffed or the “value proposition” hospice can provide, this is the series for you. Now, more than ever, it is important to be viewed as a valuable partner in their respective missions. With this series, you will become better prepared to show your patients, families, and communities the difference hospice and home health can make.

You may register for the entire series or individual sessions.

Session Dates & Descriptions

Building Strong & Compliant Skilled-Nursing Partnerships

The skilled-nursing facility (SNF) environment is changing in meaningful ways in 2019. Reimbursement requirements are easing the documentation requirements. PDPM (Patient Driven Payment Model) will be implemented, which will be a new classification system. In addition, the SNF Value-Based Purchasing (VBP) Program’s 30-day all-cause readmissions will start for the fiscal year 2019 (starting in October 2018). This program will address SNF reimbursements, regulations, and staff. What are their needs and struggles? How can we provide solutions and drive value to the nursing facilities? This session will benefit staff newly involved with SNFs and others who want more information.

Building Strong & Compliant Hospital Partnerships

The hospital environment has faced many challenges in recent years and hospice and home health providers are becoming a strong partner to the acute-care sector. This webinar will delve into the organizational structure, hospital reimbursement, and challenges facing hospital leadership. Why is it important for providers to understand hospitals’ concerns around data and analytics, populations health services, value-based payments, operational effectiveness, and cybersecurity? How can your organization become the partner of choice for a hospital? This session is designed for staff just getting oriented to the hospital environment and identifying the value proposition of your organization.

Building Strong & Compliant Assisted-Living Partnerships

The number of assisted-living communities will continue to explode within the senior-living sector. More and more providers will enter service partnerships to reduce duplication, create efficiencies, and increase market share. Assisted-living communities need to think carefully about the services they offer and the providers they use in order to ensure residents experience the best possible quality of care in home health or end-of-life care. This session will provide an overview of assisted-living-community services and challenges. Learn how to position your organization as a true outcome-focused partner.
