One in four Medicare patients hospitalized for acute medical illness is discharged to a skilled-nursing facility or an assisted-living facility. Approximately 23% of these patients are readmitted to the hospital within 30 days. The care transition from hospital to skilled-nursing or assisted-living environments is often marked by disruptions in care and poor communication between hospital and post-acute providers. Hospice and home health providers can make a difference and position themselves as the provider of choice for all three parts of this healthcare continuum.
This series will provide a comprehensive overview of each environment, identify their needs, and address how home health and hospice can manage their challenges. If you are unsure how each type of organization is led and staffed or the “value proposition” hospice can provide, this is the series for you. Building Strong & Compliant Skilled-Nursing Partnerships
The skilled-nursing facility (SNF) environment is changing in meaningful ways in 2019. Reimbursement requirements are easing the documentation requirements. PDPM (Patient Driven Payment Model) will be implemented, which will be a new classification system. In addition, the SNF Value-Based Purchasing (VBP) Program’s 30-day all-cause readmissions will start for the fiscal year 2019 (starting in October 2018). This program will address SNF reimbursements, regulations, and staff. What are their needs and struggles? How can we provide solutions and drive value to the nursing facilities? This session will benefit staff newly involved with SNFs and others who want more information.
